Registration Information
The Patient Registration Department of Plantation General Hospital sincerely welcomes you as a patient. We are pleased that you have chosen to receive medical services from our facility. We are committed to making your hospital visit as comfortable and convenient as possible.

The Admitting department is located at the Main Entrance of the hospital. All scheduled admissions must stop at the front reception desk directly inside the Main Entrance doors before proceeding to the Admitting office.
The hours of operation are Monday through Friday, 6:00 a.m. to 5:00 p.m.

EMERGENCY ROOM REGISTRATION:
The ER Registration department is located inside the Emergency Room entrance of the hospital that is located on the West side of the hospital. All Emergency Room patients must stop in the Triage area to be triaged by a nurse prior to registration.
After regular business hours, all scheduled patients (inpatient and outpatient) should report to the ER Registration department.

REQUIREMENTS FOR ALL REGISTRATIONS:
Upon arrival in the Registration department, you will be asked to verify all demographic and insurance information. You will be required to provide the following information:
- Health insurance cards and information including address, telephone number, and policy number
- Photographic identification card
- Social Security card
- A valid prescription or physician order for all tests
- Referral or authorization numbers, if required by your insurance
- A note giving "permission to treat" from a parent or legal guardian if the minor will be unaccompanied by the responsible party.
You will also be required to sign the Conditions and Consent for Treatment form at time of registration. Medicare recipients will also be asked to complete a Medicare Secondary Payer Questionnaire and sign an Important Notice from Medicare form.
ADVANCED DIRECTIVES
Upon admission to Plantation General Hospital, each inpatient is provided, upon request, information regarding medical advanced directives. If you would like information before you arrive at the facility, please contact the Admitting office during regular business hours.
DEPOSITS/COPAYMENTS
Patients are responsible for all financial liabilities including co-payments, deductibles, and out-of-pocket expenses at the time of registration or at the time of discharge for emergency admissions. If the exact dollar amount has not been determined, you will be asked to make a deposit and will be billed for any additional money owed.
PRE-REGISTRATION Pre-registration is available to all scheduled patients prior to the date of service to expedite the check-in process. During the pre-registration process, all registration information is obtained over the telephone. When you arrive at the facility, you will be required to provide your insurance card(s) and sign the required forms before going to the department.
MATERNITY ADMISSIONS
Pre-registration:
Pre-registration forms are available through our Admitting department or through your OB/GYN's office. These forms gather basic demographic and financial information and allow us to ensure that you are pre-registered and authorized for services prior to your admission.
Pre-registration forms should be completed at the beginning of the second trimester or at the time you select Plantation General Hospital for your delivery. Completed forms can be mailed or dropped off at the Admitting Department.
The following steps will help to ensure that your registration is performed in an efficient manner:
- Before you arrive at the facility for tests or procedures, you may want to contact your insurance company to obtain pre-certification and benefits information.
- Please verify with your physician's office that they have obtained the necessary authorization and referrals for your tests or procedure. The physician office must initiate the process of authorization.
- You may be required by your insurance company to notify your Primary Care Physician that you will be treated at Plantation General Hospital.
